How it Works

How does cliexa work?

Patients remotely monitor their real-time progress from their phones or tablets, which is then sent to their clinicians with a reliable, objective way to quantify their disease activity scores. This data is then pushed through our cliexa-Cloud and into the providers existing EMR system. For more information on this process, please see our solution.

Who pays for cliexa?

Clinics pay for the cliexa platform to take advantage of automated compliance, remote patient monitoring, and other solutions. Costs may vary on a per clinic basis.

What else is cliexa used for?

In addition to serving as a chronic care management platform, cliexa is used for clinical trials, processing digital forms and sending them directly to an existing EMR system, and for remotely monitoring patients progress in their disease state.

What EMR companies can cliexa integrate with?

cliexa has the capacity to integrate with any existing EMR system, including some of the larger companies as follows:

Which practices and specialties can cliexa be implemented?

  1. Rheumatology Clinics: Chronic Pain, Rheumatoid Arthritis, Fibromyalgia
  2. Pain Management: Chronic Pain, Fibromyalgia
  3. Primary Care Clinics: Depression, Alcohol Abuse, Tobacco Usage, Chronic Pain, Annual Wellness Monitoring, Medication Reconciliation
  4. Integrated Pain Clinics:  Chronic Pain, Arthritis, Chronic Fatigue, IBS and mental health

How could using a PRO collection tool like cliexa improve the accuracy and reproducibility of data?

Patient-Reported Outcomes measure any aspect of a patient’s health status directly from the patient, without the interpretation of the patient’s responses by a physician or anyone else. This inherently improves the accuracy of how and why the patient is feeling a specific way and can be translated into disease activity scores to quantify the severity of the disease.

What is PROMIS?

PROMIS stands for Patient‐Reported Outcomes Measurement Information System®.  This system of quantification provides the following benefits for physicians and clinics:

  • Measures to assess physical, mental, and social health
  • Relevant across multiple conditions
  • Psychometrically sound, developed and evaluated using state‐of‐the science methods (item response theory)
  • Developed to cover full range of domain
  • Administration options available
  • Scored on one standardized metric

How does cliexa adopt the PROMIS model into their platform?

cliexa adopts the PROMIS model into the platform by providing clinically validated assessments with disease activity scoring models to push quantifiable data to the fully integrated EMR system of clinics. This enables to receive the PROs on a phone or tablet to visualize each patient’s disease activity over periods of time.

How are we working to alleviate some of the external pressures on the Physician-Patient relationship?

cliexa has identified the following external pressures on the physician-patient relationship: Physician Burnout, Compressed Clinical Grids, EMR Reporting, Increasing Costs, Greater Medical Complexity, Variable Medical knowledge, Reduced Reimbursement, Advertisement/Marketing. We are addressing these issues through the application of PROs. Physicians found PROs a helpful mechanism for shared decision making in the right setting with the right workflow. These lead to actionable endpoints that benefit the physician-patient relationship through improved time management, a shared platform for patients and physicians to track and visualize their disease activity, and streamlined documentation.

How does cliexa collect Patient Reported Outcomes (PROs)?

cliexa collects Patient Reported Outcomes (PROs) through clinically validated assessments, customized to the preference of the clinic/care facility, remotely taken by patients prior to their visit with a physician. These assessments then provide physicians a disease activity score, providing quantifiable metrics on the severity of their disease or state of health.

Why is it beneficial to digitize and automate PROs?

PROs are nothing new in the healthcare industry; but the idea of patients remotely providing these outcomes to physicians over set periods of time. Patient-centered data collection outside of traditional clinical is the next frontier of modern healthcare. By digitizing and automating PROs, both patients and physicians benefit financially and through effective time management.

How does cliexa address the challenges of administration and integration of PROs?

cliexa provides a customizable solution to EMR integration across all specialties. Patients fill out their assessments and track their real-time progress from their phones or tablets. Clinicians are provided with a reliable, objective way to quantify disease activity. This real-time, actionable data helps expand preventive and responsive approaches without increasing costs. For multimorbid patients, physicians can visualize the data side-by-side in one platform, providing a simplified treatment process for both sides.

What are patients concerns with PROs?

We have identified the following concerns of patients with PROs: the burden of the questionnaire, frequency of data collection, location for completion, appropriate literacy level, physical/cognitive impairment, privacy concerns, value to “me”, and uncertainty of use by clinicians.

How does cliexa address these concerns?

cliexa addresses these concerns through the ease of our platform. We provide customized assessments to ensure the patients will provide accurate and meaningful answers, such as using a visual body map to track chronic pain. These assessments can be completed with the help of family and friends, providing immediate visual data that is interpretable by patients and physicians alike. The data security we provide is the same as you would receive in the hospital, with your physician being the only one with access to your secure information as we are HIPAA compliant throughout our platform.

What are physicians concerns with PROs?

We have identified the following concerns of physicians with PROs: the reluctance to change, accountability, extra effort or time required, incorporation into existing workflow, theoretical benefits, how to use the data best, and the capability to handle uncovered issues.

How does cliexa address these concerns?

cliexa addresses these concerns through our seamless integration into the existing workflow. Customizable assessments and questionnaires configure easily to your EMR system. Our cliexa Cloud pushes the data reported by the patients directly to your EMR with a customizable interface based on your preferences. The platform fits in with no disruption of your workflow and takes about one day to setup with no IT commitment.  The data can be sent directly to physicians in file formats of your preference to confirm data quality prior to full integration based on your preferences.

What are Healthcare Systems concerns with PROs?

We have identified the following concerns of Healthcare Systems with PROs: the required capital investment, clinician engagement, data management needs, integration of data, privacy concerns, and the management of critical/missing data.

How does cliexa address these concerns?

cliexa addresses these concerns through a structure of reimbursements and seamless integration. While investment is one of the key concerns, our platform links providers and payors to streamline the process of CMS and payor reimbursements. We have developed an ROI calculator on the value of our platform for those who choose to opt into our reimbursement structure, as shown here, which demonstrates the value of our platform if solely based on capital investment into healthcare systems. Clinician engagement and management needs are addressed by focusing our attention on time management and easing the process in the interest of the clinicians.

What are the concerns over implementation?

We have identified the following concerns over implementation with PROs: How PROs are selected, when they are collected, how they are integrated with the existing EMR system, what the training plan is for staff, and which outcomes are most important?

How does cliexa address these concerns?

Customizability is how cliexa addresses all the concerns over implementation. We structure our platform to the needs of the clinicians, the patients, and the healthcare system through our own team without the need for an on-site IT team. The staff can be trained in one day, and implementation itself is minimally invasive, leading to ease throughout our implementation process of pilots or full implementation

What if I change physicians or relocate to a new location?

Unlike most patient platforms, cliexa is independent of specific clinics or primary care offices. By having the past history of your chronic care treatment within the application, it makes the switch over to a new provider simpler by updating them on your chronic care treatment plan.

Do patients have access to the same versions of software as the clinics?

The cliexa platforms for patients and providers have access to the same assessments, but the set-up and interfaces are structurally different from one another as expected.

Can patients track their diseases before their visits, and will the clinic be ready with the information on arrival? Or does the patient still need to use the software in the clinic?

This depends on how clinics will manage and set-up cliexa in their system, but all of these are possible. Clinics can remotely monitor patients or monitor them solely in the clinic to the wishes of the functionality for the clinic.

How do we push software updates to the clinics or How do we maintain the updated software to the clinics?

We push updates automatically by remote device management managed by cliexa, which will automatically update the software in the clinics and on patients mobile devices or tablets.
